We Accept GP Chronic Condition Management Plan Referrals

At Living Chiropractic, we accept patients referred for chiropractic care under a GP Chronic Condition Management Plan (GPCCMP).

If you have an eligible chronic condition, your GP may recommend chiropractic care as part of your management plan. This may allow you to receive a Medicare rebate for eligible chiropractic appointments.

A GPCCMP is designed to help people with ongoing health conditions access coordinated care from their GP and other health professionals.

How long does my referral last?

A referral may state how long it is valid.

If your GP does not specify a timeframe, a GPCCMP allied health referral is generally valid for 18 months from the date of your first service under that referral.

To continue accessing Medicare-supported allied health services, your GPCCMP must also have been prepared or reviewed within the previous 18 months.

What is a chronic condition?

For the purposes of a GP Chronic Condition Management Plan, a chronic condition is a medical condition that has been, or is likely to be, present for at least six months, or is terminal.

There is no set list of conditions that qualify. Your GP will decide whether a GPCCMP is appropriate for you based on your individual health needs.

Chronic conditions may include conditions such as:

  • Chronic musculoskeletal pain
  • Arthritis
  • Diabetes
  • Heart disease
  • Chronic respiratory conditions such as asthma or emphysema
  • Chronic kidney disease
  • Other long-term health conditions

Your GP remains responsible for deciding whether you are eligible and whether allied health care, including chiropractic care, should form part of your plan.

What is a GP Chronic Condition Management Plan?

A GP Chronic Condition Management Plan (GPCCMP) is prepared with your GP to help coordinate the management of an ongoing health condition.

The plan may include:

  • Your health concerns and management goals
  • Treatments or strategies that may help you manage your condition
  • Other health professionals involved in your care
  • Referrals to appropriate allied health services
  • Plans for reviewing your progress

Your GP will discuss the plan with you and determine which health professionals may be appropriate to include in your care.

Can chiropractic care be included?

Yes. Chiropractic is one of the allied health professions eligible under the GPCCMP Medicare arrangements.

If your GP believes chiropractic care is clinically appropriate for the management of your chronic condition, they can provide you with a referral for chiropractic services.

You must have a valid GP referral before Medicare can provide a rebate for your chiropractic appointment.

How many visits can I claim?

Eligible patients can generally access up to five Medicare-rebated individual allied health services per calendar year, from 1 January to 31 December. These five appointments are shared across all eligible allied health services.

For example, your GPCCMP could include:

  • 5 chiropractic appointments or 3 chiropractic appointments and 2 dietitian appointments

or another combination of eligible allied health services.

Unused visits do not carry over into the following year. Your GP does not necessarily have to specify the number of chiropractic appointments on your referral.

Which allied health services can be included?

Depending on your individual needs, your GP may refer you to eligible health professionals including:

  • Chiropractors
  • Podiatrists
  • Exercise physiologists
  • Occupational therapists
  • Psychologists
  • Aboriginal and Torres Strait Islander health workers and health practitioners
  • Physiotherapists
  • Dietitians
  • Diabetes educators
  • Osteopaths
  • Mental health workers
  • Speech pathologists/Audiologists

Claiming your Medicare rebate at Living Chiropractic

Where available, we can lodge your Medicare claim electronically after you have paid for your appointment.

For an immediate rebate

Bring:

  • Your current Medicare card; and
  • A debit/EFTPOS card linked to a bank account.

Once the Medicare claim is processed, the rebate can usually be deposited into the linked bank account almost immediately.

The Medicare rebate cannot be paid back onto a credit card through Medicare Easyclaim.

Don’t have a debit or savings card with you?

That’s okay.

We can provide or email you your paid invoice/receipt and you can submit your Medicare claim through your Medicare online account linked to myGov.

Medicare will then deposit your rebate into your nominated bank account.

Do I receive free chiropractic visits?

No. GPCCMP appointments at Living Chiropractic are not bulk billed.

You will pay the normal consultation fee in full on the day of your appointment. Medicare then provides a rebate towards the cost of the consultation, provided you meet the eligibility and referral requirements.

The current Medicare benefit for an eligible face-to-face chiropractic service under MBS Item 10964 is $63.40. This means there will usually be an out-of-pocket gap between our consultation fee and the Medicare rebate.

Will Living Chiropractic communicate with my GP?

Yes. Chiropractors providing Medicare-funded services under a GPCCMP have reporting responsibilities to the referring medical practitioner.

We are required to provide your referring practitioner with a written report following:

  • The first service under the referral
  • The last service under the referral
  • A single service where it is the only service under the referral
  • Other occasions where there is clinically relevant information your referring practitioner would reasonably expect to receive

This helps keep your GP informed about your assessment, care and progress.

Ask your usual GP whether a GP Chronic Condition Management Plan may be suitable for you.

 

If chiropractic care may be appropriate for your condition, ask your GP about a referral for chiropractic services.

Let our reception team know that you have a GPCCMP referral when making your appointment.

Please provide us with your GP referral before or at your first eligible consultation so we can confirm the necessary information is available.

Your chiropractor will assess you and determine what care is clinically appropriate for your individual presentation.

Frequently Asked Questions

The Medicare chronic condition arrangements changed on 1 July 2025.

The current system is called the GP Chronic Condition Management Plan (GPCCMP). Older GP Management Plans and Team Care Arrangements created before 1 July 2025 have transitional arrangements in place.
