When severe sciatica goes untreated, the muscles in the affected leg can weaken or reduce in size over time—especially the calf, gluteal or quadriceps muscles. This may indicate nerve damage and, in rare cases, can become a surgical emergency if there is progressive neurological loss.
According to a 2020 review in The New England Journal of Medicine, surgery may be considered if there is:
Loss of bladder or bowel control (cauda equina syndrome)
